Our nurse care guide is there to answer questions about accessing benefits, managing your medications, qualifying for community services, or to guide you through the benefits and programs you have available to you.
You pay nothing for benefits like:
Healthy Living Allowance
$220 allowance per month to use toward eligible items, such as over-the-counter (OTC) items, fitness memberships and classes, groceries**, hygiene products**, rent** and utilities**. Debit card will be provided
Dental
$0 copay* for 2 preventive dental visits per year. Up to $3,000 allowance per year for other preventive and comprehensive dental services, such as implants, dentures, crowns and bridges
Vision
$0 copay* for an annual vision exam. Up to $500 allowance per year for lenses, frames, contacts, and eyewear upgrades
Hearing
$0 copay* for an annual hearing exam with unlimited hearing aid fittings and evaluations. Up to $2,750 allowance per year for prescription hearing aids for both ears combined
Transportation
$0 copay* for up to 50 one-way rides per year to plan-approved, health-related** locations
**Some members may be eligible to use 20 of the 50 one-way rides to any location. Each ride is limited to 30 miles
PART D Prescription Drugs
All benefits listed will begin on January 1, 2027. *$0 copay for these extra benefits, up to the annual limit. **The benefits mentioned are a part of special supplemental program for the chronically ill. Not all members qualify. (All categories except QDWI.)
