PruittHealth is proud to offer a health plan specially designed for our members. PruittHealth Premier (HMO SNP) is a Special Needs Plan contracted with Medicare to provide you with more personalized, compassionate clinical care and services for our PruittHealth Premier members.
Our PruittHealth Premier plan covers everything that Original Medicare covers under Parts A and B, and we cover Part D prescription drugs.
Plus, we cover extra benefits including vision, hearing, routine foot care, preventive care, and transportation. You pay nothing for these extra benefits!
You can join PruittHealth Premier (HMO SNP) if you have Medicare Part A (Hospital Insurance) and Medicare Part B (Medical Insurance), and meet PruittHealth Premier’s eligibility requirements.
If you meet PruittHealth Premier’s eligibility criteria, you can enroll at any time, effective the first of the following month.

PruittHealth Premier has implemented a Compliance Hotline. The Compliance Hotline provides a mechanism for callers to report activity related to known or suspected non-compliance with the law or PruittHealth Premier’s Policy. All calls to the Compliance Hotline will be treated as confidential and private to the fullest extent possible. Whether reporting by telephone or in writing, please provide as much detail as possible, including, but not limited to, names, dates, times, locations, and the specific conduct you feel may violate the law or PruittHealth Premier Policy. No individual making a good faith report of a suspected violation shall be retaliated against. However, any individual who knowingly makes a false allegation shall be subject to disciplinary action in accordance with PruittHealth Premier Policy.
